He had to be airlifted to hospital following a high speed smash at Dunsfold Park Aerodrome.
A trailer for his new documentary - titled 'Flintoff' - showed footage of his accident for the first time.
Speaking in his documentary, the 47-year-old said that he thought he had died.
"I thought I was dead, because I was conscious but I couldn't see anything," he recalled.

"I was thinking, is that it? Is that it? You know what I mean? Just black for the rest of my days?"
Flintoff added: "My hat came over my eyes - so I pulled my hat up and I thought, no I'm not [dead], I'm on the Top Gear track, this is not heaven."
Gruesome photos show the disfigurement of his face upon arrival at the hospital as well as during and after five hours of surgery.

Jahrad Haq, who operated on Flintoff, admits the injuries were among the most severe he'd ever dealt with in his career.

In the documentary, Haq revealed: "He lost a really significant portion of his upper lip - the skin and some of the underlying muscle - and also his lower lip.”
He added: “His soft tissue injuries were very complex. It's very unusual that you lose soft tissue, that you lose skin, and he'd lost a really significant portion of his upper lip, the skin and some of the underlying muscle and also his lower lip."
